You must be the age of majority in your province or territory of residence to place an order. By ordering, you confirm that the CPAP pressure settings and any prescription information you provide are accurate and that you are authorised to use a CPAP device.

Each FormFit™ mask is custom-manufactured from your individual facial scan and is made to order specifically for you. Because the product is personalised, it cannot be resold or used by anyone else, and custom orders are generally non-returnable except where the product is defective. Please see our return and warranty information below.

The accuracy of your mask depends on the quality of the scan you submit and the CPAP pressure settings you provide. Following our scanning instructions carefully helps ensure the best possible fit.

All prices are shown in Canadian dollars and may change without notice. We reserve the right to refuse or cancel any order, including where we suspect an error in pricing or product information, or where we are unable to verify the details you provided. Payment is processed securely through our payment provider at the time you place your order.

We currently ship within Canada only. Estimated production and delivery times are provided as a guide and are not guaranteed. Risk of loss passes to you once the order is handed to the carrier.

Because FormFit™ masks are custom-made, we accept returns only for products that arrive defective or damaged. If you believe your mask is defective, contact us promptly so we can assist with an assessment or warranty claim with the manufacturer. Shipping costs for returns or warranty claims may be your responsibility.

FormFit™ is a CPAP mask and accessory, not a treatment plan. Dreamsleep does not provide medical advice, diagnosis, clinical monitoring, or CPAP therapy aftercare. Always follow the instructions of your prescribing clinician and consult them with any questions about your therapy or pressure settings. The information on this website is provided for general purposes only and should not be relied on as your sole basis for any health decision.

These Terms are governed by the laws of the Province of Alberta and the federal laws of Canada applicable therein. Any dispute will be subject to the exclusive jurisdiction of the courts of Alberta.
